Ein Massenanfall von Verletzten stellt eine aussergew\u00f6hnliche Belastungssituation f\u00fcr jede Notfallstation dar. Unterst\u00fctzung und Hilfestellung ist hierbei unverzichtbar. Im Juni 2022 hat daher die Interessensvertretung der Notfallpflege Schweiz (Notfallpflege.ch) ein aktuelles Positionspapier zum Thema Massenanfall von Verletzten (MANV) ver\u00f6ffentlicht.\u00a0 Den im Positionspapier beschriebene Algorithmus haben wir gemeinsam mit Dirk Becker (Co-Autor des Positionspapiers) und dem Institut f\u00fcr Notfallmedizin des Stadtspitals Z\u00fcrich Triemli im KISIM implementiert.
<\/span><\/p>

Bei einem Massenanfall von Verletzten werden die personellen und r\u00e4umlichen Ressourcen von Rettungsdiensten und Notfallstationen an die Grenzen ihrer Leistungsf\u00e4higkeit gebracht. Ursachen k\u00f6nnen schwere Naturkatastrophen, Zugunf\u00e4lle oder auch Terrorattentate sein. Unabh\u00e4ngig von den Ursachen handelt es sich also um einen Notfall mit einer grossen Anzahl von Verletzten und\/oder Erkrankten.<\/p>

Um die Ressourcen der Rettungsdienste und Spit\u00e4ler m\u00f6glichst effizient nutzen zu k\u00f6nnen und um eine rasche Wiederherstellung einer ad\u00e4quaten Versorgung zu erreichen, werden an verschiedenen Stellen MANV-Konzepte ben\u00f6tigt. Ein wichtiger Aspekt ist dabei die schnelle und ad\u00e4quate Versorgung von Verletzten oder Erkrankten in den Notfallstationen. Hier unterst\u00fctzt der MANV-H Algorithmus.<\/p>

Eine schnelle Erstbeurteilung und Einteilung aller Patienten in verschiedene Dringlichkeitsstufen (Triage) durch triagierende Fachpersonen (\u00e4rztlicher Dienst oder Notfallpflege) ist dabei entscheidend um eine m\u00f6glichst gute Patientenversorgung zu erreichen.\u00a0<\/p>

Als Hilfestellung zur korrekten Einteilung in die Triagekategorien werden verschiedene Kriterien \/ klinische Zeichen abgefragt, die die triagierenden Notfallspezialisten mit wenigen Klicks zur richtigen Triagekategorie f\u00fchren. Gleichzeitig kann je Triagekategorie eine Raumzuteilung hinterlegt werden, damit die Patienten von der Triagestation zu den korrekten r\u00e4umlichen Ressourcen disponiert werden k\u00f6nnen. Um die gewohnten und eingespielten Prozesse m\u00f6glichst auszun\u00fctzen, wurde der MANV-H Algorithmus im gleichen Arbeitsoberfl\u00e4che wie das regul\u00e4re Triagesystem implementiert. Zur Disposition der R\u00e4umlichkeiten, wird eine erweiterte Kalenderdarstellung genutzt. Patienten, die zum Zeitpunkt des MANV bereits in Behandlung sind, bleiben in dieser Kalender-Ansicht sichtbar. Die Behandlung und Dokumentation kann f\u00fcr diese Patienten im KISIM ohne Unterbruch fortgef\u00fchrt werden.\u00a0<\/p>

Selbstverst\u00e4ndlich hoffen wir, dass ein Massenanfall ein seltenes Ereignis bleibt. Wir sind froh, dass wir im Stadtspital Z\u00fcrich einen Implementationspartner haben, mit dem wir unser Produkt f\u00fcr solche Ausnahmesituationen erfolgreich ausbauen konnten.<\/p>
